Job summary

Connecting Care is seeking a licensed practical nurse to provide leadership and coordinate resident services at Diamond Spring Lodge in Redwater, Alberta. You will work with the care team, residents, families, and health partners to promote well-being and person-directed care with dignity and compassion.

As an LPN, you will oversee nursing and care tasks, support health care aides, document care and incidents, ensure medication safety, and participate in annual education and regulatory

Qualifications

  • Graduate of a recognized LPN program and in good standing.
  • Registered with CLHA and eligible to practice in Alberta.
  • Strong organization and communication skills; team leadership.
  • Ability to lift up to 50 pounds and withstand standing/walking durations.

Role Overview

As a Licensed Practical Nurse (LPN), you provide leadership and direction for the community's environment. In collaboration with Residents, the Care Team (including residents’ families, external health care professionals, and our care partners), you coordinate and evaluate Resident services while ensuring excellent hospitality. You model a healthy community culture aligned with the Mission, Vision, and Core Values, fostering meaningful relationships, individual choice, and a sense of belonging for all.

The LPN oversees all aspects of nursing and care for Residents based on clinical best practices. As a key leader, you work with service areas, the General Manager, Supervisors, and Health Services partners to promote well-being through a person-directed approach that honors residents’ unique needs and preferences.

LPN - 1 Part Time FTE 0.5-Day Shift; LPN - 1 Casual

Key Accountabilities
  • Support health care aides with resident care needs, including assisting with transfers, monitoring, and documenting medication assistance for the MAP competency program. Focus on building trust and teamwork to ensure exceptional care.
  • Assist with non-clinical tasks such as laundry, garbage removal, attending resident events, and serving meals to support a positive resident experience.
  • Delegate and allocate health care aide tasks and work assignments as required.
  • Support urgent relief scheduling of LPNs, health care aides, and care aides as needed.
  • Lead the response to emergency codes, medical and nonmedical incidents within the community.
  • Provide documentation of incidents, safety concerns, audit results, resident and family concerns, care partner concerns, and other regulatory reporting requirements.
  • Document residents’ individualized priorities, target dates, strategies, outcomes, interventions, assessments, and follow-ups; ensure life stories are reflected alongside clinical details.
  • Complete required audits of charts, wound care binders, medication drawers, bath temperature sheets, and other items as assigned to ensure quality and compliance.
  • Ensure supply, control, and safekeeping of medications and narcotics in line with wellness strategies and LPN best practices.
  • Track and request resident supply levels from family or appropriate providers to prevent disruptions.
  • Provide medication assistance to residents in accordance with Wellness Strategies and regulatory practices; involve residents in discussions about their medications when appropriate.
  • Identify health, safety, or maintenance issues and report them as required.
  • Ensure appropriate implementation of Infection Prevention and Control practices as per Environmental Public Health guidelines.
  • Monitor the workplace environment and uphold safe work practices.
  • Provide impartial support, education, and relationship-building to nurture understanding and respect among residents, families, visitors, and care partners; foster connections that promote community and joy.
  • Offer clear and concise feedback to Wellness Manager/Supervisor on opportunities for improvement.
  • Complete annual mandatory education required by provincial regulations and the organization.
Required Education
  • Grade 12 Diploma or recognized equivalent; fluent in English.
  • Graduate of a recognized LPN program.
  • Registered and in good standing with the College of Licensed Practical Nurses and Health Care Aides of Alberta (CLHA).
  • Valid Certification in First Aid and CPR.
  • Current clear criminal record check with vulnerable sector check.

Additional Education and Skills
  • Completion of applicable courses (e.g., Preventing Resident Abuse, Customer Service, Infection Control, Back Care, Dementia, WHMIS).
  • Familiarity with the Eden Alternative Philosophy and the Domains of Well-Being is preferred.
