Job summary

Penn Highlands Healthcare is hiring a Licensed Practical Nurse to support call center functions and care management in a transitional and chronic disease management setting. You will assist in developing and evaluating patient care in the office and coordinate with other health care providers under physician supervision.

The LPN will act as a bridge between physicians, mid-level providers and external health care teams, ensuring seamless communication and patient care delivery.

Qualifications

• High school diploma or equivalent.
• Current LPN licensure in PA.
• Regulatory knowledge of Medicare/Medicaid/Blue Shield preferred.
• CPR certification within first year of hire required.
• Two years of office experience preferred.
• Familiarity with Microsoft applications preferred.

He/she will be required to coordinate the care with other services or outside health care providers. Qualifications

• High school diploma or equivalent required
• Current LPN licensure in the state of Pennsylvania required
• Knowledge in regulatory requirements of third party payors such as Medicare, Medicaid and Blue Shield preferred
• CPR certification from the American Heart Association within the first year of hire required
• Two years of office experience preferred.
• Familiarity with Microsoft applications preferred
