Job summary

West Coast SeniorsHousing Management is seeking a Licensed Practical Nurse (LPN) to coordinate and deliver resident-centered care in a full-time permanent role. The LPN will lead care delivery, liaise with physicians and families, and supervise care staff in a community setting.

Candidates must hold an active BC LPN license, complete RAI competency testing, and pass a vulnerable sector check. Salary ranges are provided, with a signing bonus and relocation assistance available under certain

Qualifications

  • Graduate of an approved Practical Nursing program and hold a current LPN license with BC College of Nurses and Midwives (BCCNM).
  • Valid Occupational First Aid Level 1 certification.
  • Successful annual RAI competency testing.
  • Previous experience in a long-term care facility is an asset.
  • Must pass the vulnerable sector Criminal Record Check as per provincial guidelines (BCCNM license).
  • Ability to read, write, and speak English proficiently.
  • Computer skills including Word, Excel, and Outlook.
  • Physical ability to push/pull/lift up to 25 lbs and work with cleaning equipment.

Responsibilities

  • Be an engaged collaborative team player and support the organizational culture.
  • Assess residents’ needs and implement individualized care plans.
  • Ensure residents’ rights and holistic wellness.
  • Liaises with physicians, families, and other staff to provide quality resident-centered care.
  • Administer prescribed medications and treatments within regulatory guidelines.
  • Monitor and document changes in residents’ conditions and notify families/physicians.
  • Participate in Interdisciplinary Care Conferences and care plan reviews.
  • Assist with admissions, discharges, and maintain confidentiality.

Job description

Salary Range: $32.84 To $44.96 Hourly

This position is covered under the HEABC wage levelling. Please note the wage is currently being adjusted and topped up by the government and the displayed wage is therefore reflective of such initiative.

We are pleased to offer a $10,000 signing bonus and up to $3300 in relocation assistance for this full-time permanent role at this community. Certain conditions apply – only available for new hires or returning employees who have not worked at any Retirement Concepts communities in the past 6 months.

Aboutthe Role

The Licensed PracticalNurse (LPN) reports to the Director of Care and/or designate. This position isthe role model and team leader to an assigned comunity, and responsible for coordinatingcare delivery of the persons served. The LPN performs duties and monitorsdepartmental activities in accordance with their scope of practice, establishedstandards, regulations, policies and procedures to maximize individualized andresident-centered care.

Responsibilities
  • Be an engaged collaborative team player, developing andmaintaining professional relationships that support and strengthen theorganizational culture and purpose.
  • Assesses residents’ needs; implements and evaluates individualized resident-centered care plans.
  • Ensures residents rights are observed and adheres to aholistic philosophy of wellness.
  • Liaises with physicians, community agency representatives, families, visitors, volunteers, other disciplines and departments to ensure quality resident-centred care is delivered.
  • Assesses and treats all wounds on the unit and acts asa resource to care staff for strategies to prevent skin breakdown. Consult withRN or designate for changes in treatment strategies as needed.
  • Administers prescribed medications and treatments within the limitations of the BC College of Nurses and Midwives (BCCNM).
  • Monitors and reports changes in physical andpsychological conditions of residents. Document changes on the resident health record,notifies family and physician of changes in condition.
  • Active participant in Interdisciplinary CareConferences.
  • Participates in care plan reviews to ensure current and individualized care plans.
  • Supportsthe administrative requirements at the community in accordance with establishedstandards, including but not limited to; admissions, discharge and ensuringconfidentiality is maintained at all times.
  • Actively participates in orientation of new staff, community programs, meetings, committees and educational in-services.
  • Providescoaching and direction to care staff as needed and uses in the moment educationto improve resident care which includes completing all Point of Care (POC)documentation. Seeks assistance/support from RN or designate as needed.
  • Maintainsan up-to-date knowledge of community policies and procedures, particularlythose concerning emergency procedures including equipment management, guidanceand assistance on safe work practices.
  • Performsother related duties as required.
Qualificationsand Skills:
  • Graduate of an approved program for Practical Nursingand holds a current license with the British Columbia College of Nurses andMidwives (BCCNM) as an LPN.

  • Successful annual Resident Assessment Instrument (RAI)competency testing.
  • Previous experience in a long term care facility anasset.
  • All successful applicants must pass the vulnerablesector Criminal Record Check applicable to Provincial guidelines (see BCCNMlicense).
  • Ability to read, write, and speak English proficientlyand understand verbal and written instructions.
  • Computer skills including the ability to navigate basiccomputer programs such as Word, Excel, Outlook.
  • Ability to push, pull, stoop, crouch, lift and carry upto 25 lbs. Must be able to work with cleaners, detergents and other cleaningequipment.
