West Coast SeniorsHousing Management is seeking a Licensed Practical Nurse (LPN) to lead and coordinate care in a Langley community. The LPN reports to the Director of Care, acts as a role model, and ensures high-quality resident-centered care within regulatory standards.

Responsibilities include assessing needs, implementing care plans, wound management, med administration, and active participation in care conferences. Strong English, mobility to lift 25 lbs, and BCCNM license required.

Qualifications

  • Graduate of an approved Practical Nursing program.
  • Current license with BCCNM as an LPN.
  • RAI competency testing completed.
  • Experience in a long-term care facility is an asset.
  • Pass the vulnerable sector criminal record check as required by guidelines.
  • Ability to read, write, and speak English proficiently.
  • Computer skills including Word, Excel and Outlook.
  • Ability to lift and carry up to 25 lbs and work with cleaning supplies.

Aboutthe Role

The Licensed PracticalNurse (LPN) reports to the Director of Care and/or designate. This position isthe role model and team leader to an assigned comunity, and responsible for coordinatingcare delivery of the persons served. The LPN performs duties and monitorsdepartmental activities in accordance with their scope of practice, establishedstandards, regulations, policies and procedures to maximize individualized andresident-centred care.

Responsibilities

Include but not limited to:

  • Be an engaged collaborative team player, developing andmaintaining professional relationships that support and strengthen theorganizational culture and purpose.
  • Assesses residents’ needs; implements and evaluates individualized resident-centered care plans.
  • Ensures residents rights are observed and adheres to aholistic philosophy of wellness.
  • Liaises with physicians, community agency representatives, families, visitors, volunteers, other disciplines and departments to ensure quality resident-centred care is delivered.
  • Assesses and treats all wounds on the unit and acts asa resource to care staff for strategies to prevent skin breakdown. Consult withRN or designate for changes in treatment strategies as needed.
  • Administers prescribed medications and treatments within the limitations of the BC College of Nurses and Midwives (BCCNM).
  • Monitors and reports changes in physical andpsychological conditions of residents. Document changes on the resident health record,notifies family and physician of changes in condition.
  • Active participant in Interdisciplinary CareConferences.
  • Participates in care plan reviews to ensure current and individualized care plans.
  • Supportsthe administrative requirements at the community in accordance with establishedstandards, including but not limited to; admissions, discharge and ensuringconfidentiality is maintained at all times.
  • Actively participates in orientation of new staff, community programs, meetings, committees and educational in-services.
  • Providescoaching and direction to care staff as needed and uses in the moment educationto improve resident care which includes completing all Point of Care (POC)documentation. Seeks assistance/support from RN or designate as needed.
  • Maintainsan up-to-date knowledge of community policies and procedures, particularlythose concerning emergency procedures including equipment management, guidanceand assistance on safe work practices.
  • Performsother related duties as required.
Qualificationsand Skills:
  • Graduate of an approved program for Practical Nursingand holds a current license with the British Columbia College of Nurses andMidwives (BCCNM) as an LPN.
  • Valid Occupational First Aid Level 1 certification.
  • Successful annual Resident Assessment Instrument (RAI)competency testing.
  • Previous experience in a long term care facility anasset.
  • All successful applicants must pass the vulnerablesector Criminal Record Check applicable to Provincial guidelines (see BCCNMlicense).
  • Ability to read, write, and speak English proficientlyand understand verbal and written instructions.
  • Computer skills including the ability to navigate basiccomputer programs such as Word, Excel, Outlook.
  • Ability to push, pull, stoop, crouch, lift and carry upto 25 lbs. Must be able to work with cleaners, detergents and other cleaningequipment.
