Job summary

West Coast SeniorsHousing Management is hiring a Licensed Practical Nurse (LPN) to lead a community in delivering resident-centered care. You will coordinate care delivery, liaise with physicians and families, and supervise care staff to ensure safety and quality.

Requirements include an approved LPN program and current BCCNM license, with RAI competency testing. Prior long-term care experience is preferred. The role emphasizes teamwork, documentation, and adherence to policy.

Qualifications

  • Must be graduate of an approved Practical Nursing program.
  • Current license with BC College of Nurses and Midwives (BCCNM).
  • Successful annual RAI competency testing.
  • Previous long term care experience is an asset.
  • Must pass provincial vulnerable sector check.

Responsibilities

  • Be an engaged team player, maintaining professional relationships to support culture.
  • Assess residents’ needs; implement and evaluate individualized care plans.
  • Ensure residents’ rights and holistic wellness philosophy.
  • Liaise with physicians, families, and staff to ensure quality care.
  • Monitor wounds and advise on treatment plans; consult with RN as needed.
  • Administer medications within BCCNM guidelines.
  • Document changes in health records and notify families/physicians.
  • Participate in care conferences and care plan reviews.

Salary Range: $32.84 To $44.96 Hourly

This position is covered under the HEABC wage levelling. Please note the wage is currently being adjusted and topped up by the government and the displayed wage is therefore reflective of such initiative.

Aboutthe Role

The Licensed PracticalNurse (LPN) reports to the Director of Care and/or designate. This position isthe role model and team leader to an assigned comunity, and responsible for coordinatingcare delivery of the persons served. The LPN performs duties and monitorsdepartmental activities in accordance with their scope of practice, establishedstandards, regulations, policies and procedures to maximize individualized andresident-centered care.

Responsibilities

Include but not limited to:

  • Be an engaged collaborative team player, developing andmaintaining professional relationships that support and strengthen theorganizational culture and purpose.
  • Assesses residents’ needs; implements and evaluates individualized resident-centered care plans.
  • Ensures residents rights are observed and adheres to aholistic philosophy of wellness.
  • Liaises with physicians, community agency representatives, families, visitors, volunteers, other disciplines and departments to ensure quality resident-centred care is delivered.
  • Assesses and treats all wounds on the unit and acts asa resource to care staff for strategies to prevent skin breakdown. Consult withRN or designate for changes in treatment strategies as needed.
  • Administers prescribed medications and treatments within the limitations of the BC College of Nurses and Midwives (BCCNM).
  • Monitors and reports changes in physical andpsychological conditions of residents. Document changes on the resident health record,notifies family and physician of changes in condition.
  • Active participant in Interdisciplinary CareConferences.
  • Participates in care plan reviews to ensure current and individualized care plans.
  • Supportsthe administrative requirements at the community in accordance with establishedstandards, including but not limited to; admissions, discharge and ensuringconfidentiality is maintained at all times.
  • Actively participates in orientation of new staff, community programs, meetings, committees and educational in-services.
  • Providescoaching and direction to care staff as needed and uses in the moment educationto improve resident care which includes completing all Point of Care (POC)documentation. Seeks assistance/support from RN or designate as needed.
  • Maintainsan up-to-date knowledge of community policies and procedures, particularlythose concerning emergency procedures including equipment management, guidanceand assistance on safe work practices.
  • Performsother related duties as required.
Qualificationsand Skills:
  • Graduate of an approved program for Practical Nursingand holds a current license with the British Columbia College of Nurses andMidwives (BCCNM) as an LPN.
  • Valid Occupational First Aid Level 1 certification.
  • Successful annual Resident Assessment Instrument (RAI)competency testing.
  • Previous experience in a long term care facility anasset.
  • All successful applicants must pass the vulnerablesector Criminal Record Check applicable to Provincial guidelines (see BCCNMlicense).
  • Ability to read, write, and speak English proficientlyand understand verbal and written instructions.
  • Computer skills including the ability to navigate basiccomputer programs such as Word, Excel, Outlook.
  • Ability to push, pull, stoop, crouch, lift and carry upto 25 lbs. Must be able to work with cleaners, detergents and other cleaningequipment.
