Base pay range

CA$32.84/hr - CA$44.96/hr

This position is covered under the HEABC wage levelling. The wage is currently being adjusted and topped up by the government; the displayed wage reflects this initiative.

About The Role

The Licensed Practical Nurse (LPN) reports to the Director of Care or designate. This role models and leads a team within an assigned community, coordinating care delivery to maximize individualized and resident-centered care, in accordance with scope of practice, standards, regulations, policies, and procedures.

Responsibilities
  • Develop and maintain professional relationships supporting organizational culture.
  • Assess residents’ needs; implement and evaluate care plans.
  • Observe residents’ rights and follow a holistic wellness philosophy.
  • Liaise with physicians, families, community agencies, and staff to ensure quality care.
  • Assess and treat wounds; consult with RNs for treatment changes.
  • Administer medications and treatments within scope of practice.
  • Monitor and report changes in residents’ conditions; document and communicate changes.
  • Participate in interdisciplinary care conferences and care plan reviews.
  • Support administrative tasks such as admissions, discharges, and confidentiality.
  • Participate in orientation, community programs, meetings, and educational sessions.
  • Provide coaching to care staff; utilize in-the-moment education and complete Point of Care documentation.
  • Stay updated on policies and emergency procedures.
  • Perform other duties as required.
Qualifications and Skills
  • Graduate of an approved Practical Nursing program; current license with BCCNM.
  • Valid Occupational First Aid Level 1 certification.
  • Successful completion of annual RAI competency testing.
  • Experience in a long-term care facility is an asset.
  • Pass a vulnerable sector Criminal Record Check.
  • Proficient in English; good communication skills.
  • Basic computer skills (Word, Excel, Outlook).
  • Physical ability to lift up to 25 lbs and work with cleaning supplies.
