Job summary

Providence Health Care is seeking a Licensed Practical Nurse for Long Term Care across multiple Vancouver sites. Work onsite at Langara and PHC facilities with rotating day/evening shifts.

The role involves direct patient care, medication administration, assessments, and care planning within a supportive team. Ideal candidates will have LPN registration in BC, CPR certification, and strong clinical/communication skills.

Qualifications

• Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience
• Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant
• Current basic cardiac life support certification

Responsibilities

• Provide safe and respectful nursing care, adhering to established policies, individualized care plans, and priorities
• Conduct thorough assessments, monitor signs/symptoms, and communicate with the RN/RPN as needed
• Prepare patients for procedures, transfers, and admissions, coordinating with the interdisciplinary team
• Administer medications per protocol, monitor effects, and maintain communication with the team
• Utilize evidence-based resources to stay updated on nursing practices
• Respond to incidents such as codes, injuries, spills, and alarms, following procedures
• Assist with staff orientation, demonstrating procedures and providing support
• Address patient learning needs, provide educational materials, and make referrals
• Attend care planning meetings, provide input, and advocate for patients/residents
• Collaborate with the team to develop and evaluate individualized care plans
• Assist with discharge planning based on patient status and external supports
• Operate and clean equipment, report malfunctions, and handle hazardous materials. Perform clerical duties such as answering phones, faxing, and maintaining records
