Job summary

SE Health is hiring Casual Licensed Practical Nurses in Vancouver to provide holistic home care to clients in their communities. The LPN will work within the scope of practice to deliver compassionate nursing care, coordinate with health professionals, and participate in ongoing training.

The role supports a community-based care model with flexible scheduling and a focus on quality outcomes across Canada. A competitive total rewards package includes education bursaries and mentorship

Qualifications

  • LPN certificate and active BCCNM registration in good standing.
  • Current First Aid and CPR (Level C) certifications.
  • Valid driver’s license and access to a vehicle.
  • Immunization records including TB test within 12 months.
  • Vulnerable sector check is required on hire.
  • Availability to work days and evenings, including weekends.
  • Able to work independently or in a team with strong organizational skills.
  • Excellent interpersonal and customer service skills.
  • Basic computer/smartphone skills.
  • Physically demanding role; able to lift up to 55 pounds.

Responsibilities

  • Provide hands-on care to clients per care plan.
  • Report changes in client condition promptly.
  • Supervise and support non-regulated care partners as assigned.
  • Identify safety or maintenance issues and report.
  • Coordinate with AHS and other professionals to ensure high-quality care.
  • Participate in team meetings and training as required.
  • Chart daily using electronic charting system.
  • Perform duties safely and efficiently.

Responsibilities:
  • Provide hands-on care to residents according to the Care Plan and Community policies and procedures.
  • Report changes in resident conditions as well as any other pertinent information to the appropriate decision makers in a timely manner.
  • Assist in the monitoring of non-regulated care partners assigned to carry out the care plan, including supervising, teaching delegated tasks, coaching, and supporting as per the hospitality model.
  • Identifies health, safety or maintenance issues, and reports as required.
  • Liaising with AHS and other health care professionals, families, and any other people required in order to facilitate the highest level of resident care.
  • Actively participates in team meetings and training/learning opportunities as required.
  • Charting on daily basis using electronic charting system
  • Perform all duties in a safe and efficient manner.
Requirements:
  • Hours are 2 hours with a client once a month
  • LPN Certificate, current active registration with the BCCNM in good standing.
  • Current First Aid (Standard) / CPR Certificate (Level C)
  • A valid driver’s license and vehicle.
  • Current immunization record including (TB test completed within the past 12 months) is required upon hire
  • Current vulnerable sector check screening is required upon hire
  • Availability to work days ORevenings; includingweekends
  • Able to work independently as well as part of a team, with excellent organizational skills to manage your schedule
  • Well-developed interpersonal and customer service skills, to effectively communicate with clients and to provide companionship to those who need it
  • Effective problem-solving and conflict management skills
  • Basic computer/smartphone skills would be an asset.
  • This is a physically demanding position and requires hand, arm and shoulder dexterity, sensory ability, lifting to a maximum of approximately fifty-five (55) pounds, walking, climbing stairs, standing, bending, reaching, and frequent movement
