AgeCare Walden Heights is seeking an experienced Licensed Practical Nurse (LPN) to join our team in Calgary. You will provide planned, patient-oriented nursing care, lead in care planning, and work with an inter-professional team to support residents and their families.

Reporting to the Director of Care, you will administer medications, ensure documentation accuracy, and participate in conferences. A 0.30 FTE, temporary PT role offers rewarding, meaningful work with a values-driven culture.

Qualifications

• Practical Nurse Diploma required.
• Current registration in good standing in the applicable Province.
• 3+ years of experience with seniors’ health.
• CPR training/certification required.
• Proficient with Microsoft Office applications.
• Demonstrates leadership, coaching, and mentorship skills.
• Adheres to AgeCare values of Trust, Respect, Quality, and Teamwork.

Responsibilities

• Collaborate with an inter-professional team applying person-centered care.
• Complete and maintain residents' clinical assessments, care plans, and documentation.
• Implement and evaluate care plans; adjust as required.
• Assist with scheduling and participating in care conferences.
• Coordinate moves in/out and transfers with the Director of Care.
• Provide direct resident care, administer medications/treatments, and document per policies.

The Opportunity

AgeCare is seeking an experienced and dynamic Licensed Practical Nurse (LPN) to join our team. Reporting to the Director of Care, this role presents a remarkable opportunity for YOU to make a significant impact on the lives of our residents and their families. The LPN is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.

What You Will Do

• Work as a member of the inter-professional team as described by our Professional Practice Model applying the person-centered approach to care.
• Complete and ensure resident's clinical care assessments, care/service plans and documentation are accurate, up to date and changes communicated as required.
• Implement and evaluate the effectiveness of the care/service plans and make changes as required.
• Assist with scheduling of resident's care conferences and participate in conferences where applicable.
• Work closely with the Director of Care to facilitate resident's move in, move out and transfers.
• Provide direct resident care and medication/ treatment administration and document care in compliance with regulatory and Resident Care standards and policies and procedures.

What You Bring

• Practical Nurse Diploma.
• Current registration and membership in good standing in applicable Province.
• 3+ years of experience with seniors’ health.
• Current CPR training.
• Computer proficiency and experience with Microsoft office applications.
• Demonstrates leadership, coaching and mentorship skills.
