Job summary

AgeCare James Bay in Victoria, BC is seeking a Licensed Practical Nurse (LPN) to join our inter-professional care team. Reporting to the Director of Care, you will provide planned, patient-centered nursing care, develop and update care plans, and participate in care conferences while ensuring accurate documentation and regulatory compliance.

The role requires an active LPN license, CPR/First Aid, and experience in senior or long-term care.

Qualifications

• Licensed Practical Nurse in good standing in the applicable province.
• Experience in senior care or long-term care preferred.
• Current CPR and First Aid certification.
• Completion of a Vulnerable Sector Check (or willingness to obtain).

Responsibilities

• Work as a member of the inter-professional team applying person-centered care.
• Complete and ensure resident's clinical care assessments, care/service plans and documentation are accurate, up to date and changes communicated as required.
• Implement and evaluate the effectiveness of the care/service plans and make changes as required.
• Assist with scheduling of resident's care conferences and participate in conferences where applicable.
• Work closely with the Director of Care to facilitate resident's move in, move out and transfers.
• Provide direct resident care and medication/ treatment administration and document care in compliance with regulatory and Resident Care standards, policies, and procedures.
