Job summary

Nova Scotia Health Authority in Central Zone seeks an Interdisciplinary Clinical Leader to provide leadership and support to the primary health care clinical team, focusing on functional health and person-centered care. Qualifications

  • Bachelor's degree plus 5 years' related experience or Master's plus 3 years' related experience.
  • Licensed with the respective Nova Scotia regulatory college.
  • Excellent organizational and time management skills.
  • Experience with complex chronic health conditions and pain.
  • Experience with behaviour change counselling and self-management support.
  • Experience with leadership of multidisciplinary clinical teams.
  • Excellent interpersonal and communication skills.
  • Ability to work as a team and to prioritize under deadlines.
  • Excellent research and program evaluation skills.
  • Proficient in Microsoft Office products.
  • Familiarity with primary care EMR preferred.
  • Competencies in other languages an asset, French preferred.

Responsibilities

  • Provide leadership and support to the interdisciplinary clinical team.
  • Coordinate and deliver clinical care to patients ensuring person-centered care.
  • Foster healthy interdisciplinary team processes and community partnerships.
  • Participate in recruitment, orientation and quality improvement activities.
  • Promote evidence-based practice within Primary Health Care.

About the Opportunity

Please note: This position requires the successful incumbent to strictly adhere to Integrated Chronic Care Services (ICCS) scent-free free policy.

Reporting to the Health Services Manager, Primary Health Care, the Interdisciplinary Clinical Leader provides leadership, innovation and support to the clinical team in the provision of interdisciplinary, collaborative, person-centred care to patients with a focus on functional health. The Interdisciplinary Clinical Leader works closely with patients and families, team members (physicians, psychologists/clinical therapists, occupational therapists, physiotherapists, social workers, nurses, office staff) family physicians, community organizations, primary, secondary and tertiary health services, to promote person-centered collaborative care within the Primary Health Care portfolio.

The incumbent develops and fosters a healthy interdisciplinary team processes that develop a person and family centered continuum of care focused on functional health goals, building relationships with community based services ensuring practice is interdisciplinary and evidence based, program planning and evaluation, participating in the recruitment, selection and orientation of new team members, supporting a quality approach to care delivery, coordinating and providing clinical care to patients.

Hours
  • Permanent full-time position; 75 hours biweekly
Compensation and Benefits

$43.07 - $55.10 hourly

Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
