Job summary

Northern Health is seeking a Primary Care MHSU Clinician to support mental health and addiction care within our Primary Care Homes in Terrace. You will work as part of an interprofessional team to assess, plan, and deliver evidence-based care aligned with person-centered goals.

Responsibilities include initial and follow-up assessments, care planning, education for patients and families, and collaboration across disciplines to improve outcomes in community settings.

Qualifications

• Bachelor’s degree in Allied Health, Behavioral or Social Science field relevant to the position plus two years recent health setting experience with complex care clients.

Responsibilities

• Assess and develop initial and ongoing assessments; provide appropriate therapeutic interventions.
• Develop and implement a plan of care for mental health or addiction.
• Collaborate with interprofessional team to coordinate care across settings (physician practices, homes, schools, communities).
• Provide education and support to patients and families.
