Job summary

Qualifications

• Must be registered in good standing with the applicable Ontario regulatory bodies.
• 1+ year of recent experience in community health or related field.
• Knowledge of the health care delivery system and community resources.
• Excellent interpersonal, communication, assessment, problem-solving and decision-making skills.
• Valid driver’s license and access to a reliable vehicle.
• Proficient in a Windows environment.
• COVID-19 vaccination status required prior to start date.

Responsibilities

• In collaboration with patients and families, assess care needs and determine eligibility for services.
• Link patients with service providers.
• Coordinate and monitor care plan delivery.
• Establish a helping relationship with patients and families.
• Balance patient needs and choices with available resources while respecting values.
• Complete required documentation, reports and forms accurately.
