Job summary

A leading healthcare organization in British Columbia is seeking a dedicated Social Worker to join their Hospice Palliative Community Services team. In this role, you will provide comprehensive counseling services and assess the psychosocial needs of clients and families facing end-of-life issues. You will collaborate with an interdisciplinary team to develop care plans and ensure client autonomy in decision-making processes. A Master’s in Social Work and BC registration are required. Qualifications

  • Master’s degree in Social Work from an approved School of Social Work.
  • Two years recent, related clinical experience with palliative clients.
  • Current full registration with the BC College of Social Workers.

Responsibilities

  • Provides comprehensive therapeutic and clinical counseling services to clients.
  • Conducts social and psychosocial assessments for clients and families.
  • Follows up and provides ongoing monitoring of client situations.
  • Acts as a member of the interdisciplinary care team.
  • Promotes client autonomy in planning and decision-making.
