Job summary

Sanctum Care Group is seeking a home support worker to provide care and support to residents in a transitional care setting for pregnant women at high risk or living with HIV or substance use. The role emphasizes patient-centered harm reduction and a supportive environment to foster mother-infant bonding.

Responsibilities include assisting with personal care, medication adherence, meals, and general household duties, while maintaining safety and following medical instructions.

Qualifications

• Harm reduction and trauma-informed care are required.
• Mental health and addictions knowledge is essential.
• Cultural sensitivity is important.
• Conflict resolution and interpersonal skills are needed.
• Strong decision-making in varying situations is expected.
• CPR and First Aid certification must be active.
• Food Safety Certificate must be active.
• TLR Certificate must be active.
• Naloxone Training is an asset.

Job description

Sanctum 1.5 is a transitional care home for pregnant women who are at high risk of contracting HIV, are HIV positive or in active substance use. We offer a supportive home for women and facilitate mother-infant bonding and reduce child apprehensions at birth.

The home support worker provides care and support to clients. Clients may also be affected by mental illness, addiction, poverty, homeless or other psycho-social factors. The home support worker will embody a patient centered harm reduction care model fostering an environment of acceptance and understanding for residents.

Day Shifts - 07:30-16:00

Evening Shifts - 15:30-00:00

Night Shifts - 23:30-08:00
