Job summary

CenterWell Home Health is seeking a Home Health Registered Nurse to provide skilled nursing in patients' homes and collaborate with physicians and clinicians to support recovery and independence.

You will develop care plans, monitor progress, educate families, and maintain detailed records while coordinating with the care team and supervising Home Health Aides. A valid RN license and CPR certification are required, along with reliable transportation.

Qualifications

• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Provide skilled nursing care to patients, working onsite in their homes, including administering medications, wound care, and assessments.
• Develop and implement individualized care plans in collaboration with physicians and other professionals.
• Monitor patients' conditions and report changes.
• Educate patients and families on disease management, medications and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient progress to physician and Clinical Manager.
• Oversee Home Health Aides and ensure proper documentation.
