Job summary

Vancouver Coastal Health is seeking a Case Manager to oversee a select group of chronically ill or disabled clients in the community. You will collaborate with the physician and multidisciplinary team to assess needs, plan care, and coordinate services across the continuum.

The role emphasizes continuity of care, clinical interventions, and transitions, with a focus on client-centred education and evidence-based practice in home health settings.

Qualifications

• Current practising registration as a Registered Nurse (RN) or Registered Psychiatric Nurse (RPN) with the BC College of Nurses and Midwives (BCCNM).
• Experience in home care nursing or acute care with chronic conditions; knowledge of case management.
• Valid BC driver’s license; ability to travel within local area.

Responsibilities

• Coordinate care across the continuum within a multidisciplinary team.
• Assess, plan, implement and evaluate client goals for care and outcomes.
• Liaise with physicians, community agencies and educational resources to achieve client-centred care.
