Job summary

ParaMed, a leading home health care provider in Canada, seeks a Clinical Planner responsible for onboarding new patients requiring Personal Support Services. The role conducts assessments, develops care plans, and ensures safe operating conditions.

• Registered in good standing as an RPN/LPN with provincial bodies.
• Minimum two years’ nursing experience.

Responsibilities

• Perform initial patient care needs assessments and develop individual care plans.
• Complete initial safety assessment of patients’ living environment for safe operations.
• Provide initial personal care services for new patients.
• Advise on scheduling parameters for ongoing visits based on clinical needs.
• Inform patients/families of policies, scheduling practices, and expected care times.
• Maintain accurate patient records.
• Respect cultural needs, privacy, and confidentiality.
