Job summary

ParaMed Home Health Care is seeking a Clinical Planner to onboard new patients needing Personal Support Services within an assigned district. The role covers assessment, implementation, and evaluation of clinical and personal care interventions to establish effective care plans.

The candidate will perform initial care needs assessments, safety checks, and personal care services for new patients, while coordinating scheduling and maintaining records.

Qualifications

• Registered nursing license in good standing (RPN/LPN).
• Strong oral and written communication skills.
• Excellent organizational skills.
• Competency in patient-centered care principles.

Responsibilities

• Perform initial patient care needs assessment and create care plans.
• Complete initial safety assessment of living environment for safe work conditions.
• Initiate personal care services for new patients.
• Advise scheduling on ongoing visit parameters based on clinical needs.
• Inform patients and families of ParaMed policies and timing of care.
• Maintain accurate patient records.
• Respect cultural needs, dignity, privacy, and confidentiality.

Education

RPN/LPN registered in good standing
