Job summary

Northern Inter-Tribal Health Authority (NITHA) is seeking Home Health Aide / Community Health Representative (CHR) to provide home-based supports and outreach in Prince Albert and other location sites. The CHR acts as a liaison between providers and community members to improve access and education within culturally grounded health promotion.

What You’ll Do:

• Home visits, vitals, and basic supports within scope.
• Accompany clients to appointments; navigation and advocacy.
• Health promotion, materials, and reminders.
• Community outreach and program support.

Prerequisites:

• Grade 12 diploma — Regular Grade 12, Adult 12, or GED.
