Job summary

NITHA in Prince Albert, SK, seeks a Nurse Navigator to help clients and families navigate services, appointments, and transitions of care, removing barriers to access.

You will educate patients, coordinate care plans, book referrals, and advocate for culturally safe care across clinics, hospitals, and community programs within northern health services.

Collaboration with partner organizations and local communities will support holistic, culturally respectful care pathways.

Qualifications

• Nursing background preferred; strong system knowledge.

Overview:
