Job summary

Southern Health-Santé Sud in Steinbach is seeking a Case Coordinator for Home Care. You will work with a multidisciplinary team to assess plans, coordinate services, and monitor client needs to ensure safe, person-centered care.

The role requires a minimum of two years related experience, post-secondary education (B.N./B.S.W./RN or equivalent), active license, and strong communication, organization, and confidentiality skills.

Qualifications

• Minimum of two years current related experience.
• Post-secondary education program in nursing or social work or RN with relevant medicine/community/long-term care experience.
• Active license/registration to practice; immunization as required.

Responsibilities

• Provide clinical coordination for Home Care clients with independence as a multidisciplinary team member.
• Assess plans, coordinate, monitor and evaluate required health and human services using available resources.
• Maintain confidentiality and professional relationships with clients and partners.
