Essential Duties

• Follow care plans and report on tasks completed.
• Assist with personal care needs (bathing, dressing, etc.).
• Help with routine house cleaning, meal prep, and laundry.
• Transport clients for appointments and errands.
• Assist with medication self-administration.
• Observe and report changes in client's condition.
• Maintain confidentiality at all times.
• Attend regularly and be flexible to meet department needs.
• Follow all relevant regulations (Medicare, Medicaid, HIPAA).
• Perform other duties as assigned.

Position Requirements & Competencies

• Must be 18 years or older.
• Certified Home Health or Personal Care Aide.
• Pre-employment physical and PPD test.
• Pass a criminal background check.
• Reliable transportation; valid driver's license and proof of insurance if using a car.
• Nurturing, compassionate, and desire to care for others.
• Ability to work independently and follow instructions.
• Good communication skills.
