Job Summary:

The Home at Last (HAL) Attendant supports clients in the Home at Last Program through the hospital discharge process and assists clients with settling back in at home following discharge. The HAL Attendant collaborates with the healthcare team to meet individuals' needs, promoting independence and well-being.

Key Responsibilities:
  • Meet clients at hospital to support discharge and escort them home.
  • Observe and evaluate the individual's/family's home and care plan.
  • Assist in developing personal support care plans with the Program Coordinator.
  • Perform personal support tasks and errands as required.
  • Communicate with healthcare team regarding client needs.
  • Perform light homemaking and assist with personal care if needed.
  • Prepare meals and conduct environmental risk assessments.
Common Responsibilities:
  • Promote the goals and values of VON as an integrated community care provider.
  • Ensure a safe and healthy workplace compliant with OHS regulations.
  • Follow all policies, including confidentiality and personal information protection.
  • Collaborate with team members for service delivery.
External and Internal Relationships:
  • Engage with clients, families, Program Coordinators, discharge planners, and other healthcare providers.
Education, Designations, and Experience:
  • Secondary school education preferred.
  • Experience with diverse social and economic backgrounds preferred.
  • Current CPR and First Aid certification required.
  • Personal Support Worker or Health Care Aide certification is an asset.
  • Household management experience.
  • Experience with elderly or ill individuals.
Skill Requirements:
  • Excellent communication skills, both written and verbal.
  • Good assessment skills.
  • Knowledge of community resources.
  • Ability to work independently and in a team.
Other Requirements:
  • Access to a personal vehicle with valid driver’s license and insurance.
  • Satisfactory Criminal Records Check required at employment start.
  • Potential need for Vulnerable Sector Search and Child Abuse Registry Check.
  • French language skills are an asset in designated areas.
  • Ability to wear PPE.
Working Conditions and Physical Demands:

This role involves physical activity such as lifting, bending, reaching, and walking. Candidates should practice good body mechanics and be comfortable with tasks requiring fine hand movements in a dynamic environment.
