Job Summary:
The Home at Last (HAL) Attendant supports clients in the Home at Last Program through the hospital discharge process and assists clients with settling back in at home following discharge. The HAL Attendant collaborates with members of the healthcare team to meet the needs of individuals to attain an optimal level of independence and well-being.
Key Responsibilities:
- Meet clients at hospital to support through discharge process and escort them home.
- Observe and evaluate the individual's/family's home, personal situation, and client/family care plan.
- Contribute to the development of the personal support care plan with the help of the Program Coordinator.
- Perform personal support tasks reflecting the needs of the individual/family, assist with errands as required.
- Communicate verbally with appropriate members of the health team regarding the management of the client's/family's homemaking, physical, and emotional needs.
- Perform light homemaking for clients in their home and assist with personal care if required.
- Prepare meals and complete environmental risk assessments.

- Promote a safe and healthy workplace, ensuring conduct and activities comply with the provincial Occupational Health and Safety Act and Regulations.
- Abide by all VON policies and work practices, including confidentiality and protection of personal information policies, regulations, and practices, ensuring safeguards are in place within their role.
- Collaborate with other staff in a team approach to service delivery.
External and Internal Relationships:
- Engage frequently with clients, family members, Program Coordinators, discharge planners, and other healthcare providers regarding client plans and relevant information.
Education, Designations, and Experience:
- Secondary school education preferred.
- Experience working with people from diverse social and economic backgrounds preferred.
- Current CPR and First Aid certification required.
- Personal Support Worker or Health Care Aide certification is an asset.
- Household management experience.
- Experience working with elderly or ill individuals.
Skill Requirements:
- Excellent written and verbal communication skills.
- Good assessment skills.
- Awareness of community resources.
- Ability to follow directions and work independently or as part of a multidisciplinary team.
Other Requirements:
- Access to a personal vehicle with a valid driver's license and proof of insurance to attend client and hospital visits.
- Satisfactory Criminal Records Check required upon employment start.
- Vulnerable Sector Search and/or Child Abuse Registry Check may be required.
- Ability to speak French is an asset in French Designated areas.
- Must be able to wear Personal Protective Equipment (PPE).
Working Conditions and Physical Demands: This role requires a detail-oriented approach in a dynamic environment, involving physical activities such as lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements emphasizing good body mechanics. Tasks include walking, sitting, standing, climbing stairs, and fine hand movements throughout the day.
