Job summary

WNHAC in Kenora, Ontario, is seeking a Home and Community Care Personal Support Worker for a full-time 12-month contract. You will work with the HCC Nurse and clients to develop and monitor care plans, provide supportive care to promote independence, and ensure safe, holistic care within the community.

Home and Community Care Personal Support Worker

Serving AWZ33 & AWZ37

Full-Time (Existing Position) 12-month Contract

We provide primary health care through a range of options including prevention, assessment, diagnosis, and treatment. Our interprofessional teams include health professionals, educators and support workers representing both traditional Indigenous and contemporary models of care and using wholistic health and community development concepts.

NOTE: Indigenous traditional practices involve cultural ceremonies and burning sacred medicines including tobacco, sweetgrass, sage and cedar within the work setting.
PRIMARY DUTIES / RESPONSIBILITIES:
  • Work with the HCC Nurse and clients to establish, coordinate, and monitor health care plans to promote overall quality health.
  • Provide supportive care to clients that encourages client independence to the greatest possible extent.
  • Assist with personal care as identified in care plan, observing changes in physical condition and function and reporting changes to Home Care Nurse.
  • Assist with ambulation, transfer, and positioning.
  • Assist with meal preparation and feeding, or feed client as indicated in care plan.
  • Provide for elimination of body waste by assisting clients with toileting; prepare and care for equipment to ensure client safety and comfort and to maintain a sanitary environment.
  • Collaborate with professional team members to ensure comprehensive wholistic care for clients, making referrals and seeking and/or providing consulting support as appropriate.
  • Maintain current and accurate medical records, reports, and other documentation to ensure comprehensive, consistent care and services, and to meet all accountability requirements.
  • Active participation in organizational teams and committees.
  • Other duties as required. In extenating situations or in the event of an emergency, may be

QUALIFICATIONS:
  • Current certification as a Personal Support Worker;
  • Experience working in an Indigenous organization and ability to understand and speak Anishinaabemowin a definite asset;
  • Demonstrated ability to work independently and be an effective team member;
  • Willingness and ability to travel;
  • Valid drivers’ license and own vehicle required; copy of current drivers’ abstract required;
  • Current criminal/vulnerable persons records check required;
  • Commitment to ongoing training and professional development relevant to job requirements;
  • Proof of immunization in compliance with policy requirements is mandatory prior to start of employment;
  • Positive attitude and capacity to act as a healthy lifestyle role model
  • Certain duties of the position are physically demanding
Salary:

$47,014- $61,342 with excellent benefits, including HOOPP pension plan. Salary is based on education and experience.
