Job summary

CarePartners is seeking a Team Coordinator to ensure excellent customer service and smooth communication among patients, families, and staff. The role focuses on scheduling PSWs, nurses, and therapists to meet service delivery standards and performing related administrative duties.

The position requires post-secondary education and at least one year of related experience, with strong Microsoft Office skills and a patient-centered approach.

Qualifications

• Post-secondary education preferred and one year of job-related experience.
• Exceptional interpersonal and customer service skills.
• Proficient in Microsoft Office, especially Word and Excel.

Responsibilities

• Bridge between patients/caregivers, families, PSWs, supervisors, funders and staff.
• Receive referral/intake information from funders and private-pay clients.
• Develop and coordinate service schedules; inform workers of changes.
