Job summary

Bayshore HealthCare in Kingston, Ontario is seeking a Client Scheduling Coordinator who connects patients, families, and healthcare providers to ensure timely and appropriate home care services. You will engage with patients, families, and staff, process funder referrals, and help tailor care schedules to geographic and clinical needs.

Qualifications

• Two years of related customer service experience, preferably in healthcare.
• Strong written and spoken English communication skills.
• Ability to prioritize and manage multiple requests in a fast-paced environment.

Responsibilities

• Communicate professionally with patients, families, staff, and funder organizations.
• Process referrals and updates in the EMR (Procura).
• Review service needs with Supervisors, field staff and patients.
• Schedule care providers based on client needs, geography, and staff availability.
• Coordinate caregiver assignments and service delivery; notify patients and staff of schedules.
• Utilize scheduling tools to optimize staff availability and client care needs.
• Maintain accurate EMR records and resolve patient concerns per escalation paths.
