Qualifications

• Minimum of two years related customer service experience, preferably in healthcare.
• Strong communication skills, both written and oral in English, with tact and diplomacy.
• Advanced computer skills: Windows, Microsoft Office, EMR systems, internet.
• Ability to prioritize, multitask, and work in a fast-paced environment.

Responsibilities

• Communicate professionally with patients, families, staff and funders.
• Process funder referrals and EMR updates (Procura) quickly.
• Schedule and assign care providers based on client needs and geography.
• Coordinate caregiver assignments and ensure service delivery is consistent.
• Notify patients and staff of initial and ongoing schedules; document records accurately.
• Address patient concerns promptly and follow escalation pathways.
