CarePartners is seeking Team Coordinators to ensure smooth patient care, scheduling of PSWs, nurses and therapists, and timely administrative support. The role supports communication among patients, families and staff in Stratford, ON.

This full-time position offers a competitive salary in the CA$50,000–CA$55,000 yearly range, with a 6-month temporary contract and defined shifts including days, afternoons, evenings, weekends and weekdays.

Qualifications

• Post-secondary education is preferred.
• One year of job-related experience (administrative or customer service) is required.
• Excellent interpersonal and customer service skills.

Responsabilités

• Act as the bridge between patients, families, PSWs, and staff.
• Receive routine referrals/intake information from funders and clients.
• Coordinate service schedules and inform workers of changes.
• Enter data into the information system and generate reports.
• Handle inbound calls and email inquiries related to service.
• Perform other administrative duties as needed.
