Job summary

enCompass Community Safety Agency seeks a Healthcare Navigator to support clients in the Integrated Offender Management program. You will coordinate healthcare services, referrals, and appointments, while addressing barriers to care and stability.

You will work with Social Navigators, EPS partners, and clinicians to promote continuity of care, patient advocacy, and informed health decisions in a client-centered team.

Qualifications

• Completion of a recognized LPN, RN, or PCP education program.
• Current registration with the applicable regulatory body.
• Minimum 3 years of experience with individuals who have complex needs.

Responsibilities

• Assess and support clients' healthcare needs during onboarding and ongoing involvement in IOM/BAU.
• Coordinate referrals, appointments, and access to primary care and specialists.
• Assist with healthcare information systems (Netcare, Connect Care) to obtain records and lab results.
• Provide healthcare education and harm reduction guidance to clients.
• Maintain documentation and administrative responsibilities.

Position Summary:

Reporting to the Team Lead of Reintegration Services, the Healthcare Navigator supports the medical and healthcare needs of clients involved in the Integrated Offender Management (IOM) Program. Working collaboratively with Social Navigators and Edmonton Police Service (EPS) Diversion & Desistance Detectives, the Healthcare Navigator helps clients access appropriate healthcare services, navigate complex systems, coordinate referrals and appointments, and address barriers that may impact their health and overall stability.

The IOM Program supports individuals with complex needs through coordinated, client-centred planning using principles of High-Fidelity Wraparound and the Risk-Need-Responsivity (RNR) model. The Healthcare Navigator contributes a health-focused perspective to this work by helping identify medical and healthcare needs, connecting clients with appropriate services and resources, and supporting the development of integrated plans that respond to identified risks and needs.

Through collaboration with clients, program staff, EPS partners, healthcare providers, and community supports, the Healthcare Navigator promotes client self-efficacy, continuity of care, and sustainable positive change.

Responsibilities:

• Assess and support clients' healthcare needs during onboarding and throughout their involvement with the Integrated Offender Management (IOM) and Behavioural Assessment Unit (BAU) programs.
• Access appropriate healthcare information systems, including Netcare and Connect Care, to support care coordination and obtain relevant information such as lab results, medication profiles, pharmacy information, healthcare numbers, hospital involvement, and program availability.
• Complete and coordinate healthcare, detox, treatment, and other health-related referrals.
• Support clients in accessing primary care, specialist services, and other appropriate healthcare providers.
• Coordinate medical appointments, provide reminders and follow-up, and attend appointments to advocate for clients when required.
• Provide clients with healthcare and harm reduction education to support informed decision-making and engagement in their care.
• Provide healthcare-related consultation and guidance to the interdisciplinary team within the scope of the role.
• Complete medical assessments required to support treatment applications and other program-related healthcare needs.
• Conduct drug testing related to release conditions and admission to treatment programs, in accordance with applicable procedures and professional scope.
• Collaborate with Social Navigators, EPS Diversion & Desistance Detectives, healthcare providers, community partners, and other members of the client's support network to support coordinated service planning.
• Participate in internal and external meetings and work with program leadership to support the effective integration of healthcare services within the program.
• Support the development and delivery of healthcare and harm reduction education and participate in relevant professional development and training.
• Maintain required documentation and complete program and agency administrative responsibilities.
• Provide client transportation on a case-by-case basis when required.
• Contribute to the agency's mission, vision, and values through professional, collaborative, and client-centred practice

Qualifications / Skills:

• Completion of a recognized Licensed Practical Nurse (LPN), Registered Nurse (RN), or Primary Care Paramedic (PCP) education program.
• Current registration and good standing with the applicable professional regulatory body.
• Minimum of 3 years of relevant experience supporting individuals with complex needs and/or high-risk populations, including working with their families and support networks.
• Strong knowledge of healthcare and community resources, with an understanding of the barriers experienced by individuals impacted by homelessness, poverty, mental health concerns, and substance use.
• Strong critical thinking, ethical decision-making, accountability, organization, communication, and empathy.
• Demonstrated ability to build and maintain effective professional relationships with colleagues, healthcare providers, community partners, and other agencies.
• Experience working with Indigenous and culturally diverse individuals and families, with an understanding of the unique barriers and challenges they may experience.
• Strong teamwork and interpersonal skills, with the ability to work collaboratively within an interdisciplinary environment.
• Proficiency with Microsoft Office applications and general digital literacy.
• Valid Class 5 Alberta driver's licence (non-GDL), reliable transportation, and $2 million automobile liability coverage.

Conditions of Employment:

• Hours of work are40 hours per week, with a preferred schedule of four 10-hour shifts (Monday-Thursday). A standard five-day, 8-hour schedule (Monday-Friday) may also be considered.
• Ability to obtain and maintain Enhanced Security Clearance.
• Current Standard First Aid certificate is required.
• Completion of Agency core training in Suicide Intervention, Crisis Intervention, and De-escalation training is required.
• Will hold a valid Class 5 Alberta driver's license (non-GDL), Alberta registration, $2 Million insurance liability coverage, with the ability to operate a personal vehicle.
