Job Summary
We are seeking a dedicated and experienced
Registered Practical Nurse (RPN) to join our team as a
Health and Wellness Manager in the Newmarket Area. In this role, you will oversee and coordinate the delivery of high-quality health and wellness programs, ensuring the physical, mental, and emotional well-being of clients or residents. You will play a key role in promoting a culture of care, wellness, and safety while adhering to regulatory requirements and organizational standards.
Key Responsibilities
- Health and Wellness Program Management
- Plan, implement, and evaluate health and wellness initiatives tailored to meet the needs of clients or residents.
- Oversee the development and maintenance of individualized care plans, ensuring they align with best practices and client needs.
- Monitor and manage chronic diseases, preventative care, and health promotion activities.
- Clinical Leadership
- Supervise and mentor healthcare staff, ensuring compliance with care protocols and professional standards.
- Conduct regular audits and quality assurance checks to maintain high standards of care.
- Provide hands-on clinical care and support as needed, including administering medications and treatments.
- Collaboration and Communication
- Act as the primary liaison between healthcare providers, families, and clients to facilitate effective communication and care coordination.
- Partner with multidisciplinary teams to ensure a holistic approach to health and wellness.
- Educate clients, families, and staff on health promotion and disease prevention.
- Regulatory Compliance and Risk Management
- Ensure compliance with provincial healthcare regulations, including the College of Nurses of Ontario (CNO) standards.
- Maintain accurate and confidential medical records in accordance with privacy laws and organizational policies.
- Develop and implement strategies to minimize risks and improve client safety.
- Administrative Duties
- Schedule, coordinate, and oversee healthcare services and staff assignments.
- Manage budgets and resources related to health and wellness programs.
- Participate in recruitment, training, and performance evaluations of healthcare staff.
Qualifications
Education and Certification
- Current registration with the College of Nurses of Ontario (CNO) as an RPN in good standing.
- Additional certifications in health and wellness, geriatrics, or chronic disease management are an asset.
Experience
- Minimum of 3 years of nursing experience, preferably in community Retirement, long-term care, or wellness program management.
- Experience in a leadership or supervisory role is strongly preferred.
Skills And Competencies
