Job Title: Health and Wellness Manager (RPN)
Location: Newmarket Area

Job Summary:

We are seeking a dedicated and experienced Registered Practical Nurse (RPN) to join our team as a Health and Wellness Manager in the Newmarket Area. In this role, you will oversee and coordinate the delivery of high-quality health and wellness programs, ensuring the physical, mental, and emotional well-being of clients or residents. You will play a key role in promoting a culture of care, wellness, and safety while adhering to regulatory requirements and organizational standards.

Key Responsibilities:
1. Health and Wellness Program Management

Plan, implement, and evaluate health and wellness initiatives tailored to meet the needs of clients or residents.

Oversee the development and maintenance of individualized care plans, ensuring they align with best practices and client needs.

Monitor and manage chronic diseases, preventative care, and health promotion activities.

2. Clinical Leadership

Supervise and mentor healthcare staff, ensuring compliance with care protocols and professional standards.

Conduct regular audits and quality assurance checks to maintain high standards of care.

Provide hands-on clinical care and support as needed, including administering medications and treatments.

3. Collaboration and Communication

Act as the primary liaison between healthcare providers, families, and clients to facilitate effective communication and care coordination.

Partner with multidisciplinary teams to ensure a holistic approach to health and wellness.

Educate clients, families, and staff on health promotion and disease prevention.

4. Regulatory Compliance and Risk Management

Ensure compliance with provincial healthcare regulations, including the College of Nurses of Ontario (CNO) standards.

Maintain accurate and confidential medical records in accordance with privacy laws and organizational policies.

Develop and implement strategies to minimize risks and improve client safety.

5. Administrative Duties

Schedule, coordinate, and oversee healthcare services and staff assignments.

Manage budgets and resources related to health and wellness programs.

Participate in recruitment, training, and performance evaluations of healthcare staff.


Qualifications:
Education and Certification

Current registration with the College of Nurses of Ontario (CNO) as an RPN in good standing.

Additional certifications in health and wellness, geriatrics, or chronic disease management are an asset.

Experience

Minimum of 3 years of nursing experience, preferably in community Retirement, long-term care, or wellness program management.

Experience in a leadership or supervisory role is strongly preferred.

Skills and Competencies

Strong knowledge of health promotion, chronic disease management, and wellness practices.

Excellent leadership, organizational, and time management skills.

Exceptional communication and interpersonal abilities.

Proficiency in using electronic medical records (EMR) systems and other relevant technology.

Commitment to ongoing professional development and learning.
