Tr’ondëk Hwëch’in Government is seeking a Health Programs & Services Manager (Term) to lead the development and delivery of home and community care, elder support, and community health initiatives in Dawson City, Yukon.
Reporting to the Director of Wellness, you will coordinate case management, support citizens with complex health needs, and guide high-risk health planning while aligning with TH values, policies and priorities. Immediate start with a two-year term and competitive compensation.
Reporting to the Director of Wellness, the Health Programs and Services Manager oversees the development, implementation and delivery of home and community care, Elders support, community health, patient navigation and health-related programs and services. In addition to coordinating client case management and supporting Citizens experiencing complex health, mobility, aging, accessibility, chronic disease and acute health challenges, the incumbent leads complex and high-risk health cases and case planning, while ensuring that appropriate systems, procedures and service delivery frameworks are in place to support effective and coordinated care. The incumbent contributes to program development, funding initiatives, budget administration and community engagement activities, and liaises with governments, agencies and community partners to identify and respond to emerging health and wellness needs. The incumbent supervises and supports a team of jëje’in and ensures programs and services are delivered in accordance with TH values, legislation, policies and strategic priorities.
An Eligibility List may be established from this competition – the duration of the eligibility list may be up to 12 months. The eligibility list may be used to fill future permanent full-time, permanent part-time, temporary full-time, temporary part-time, term, or auxiliary on-call vacancies within the same department and classification based on the organizational needs by going the next highest ranked candidate until the eligibility list expires.
