Job summary

Chartwell Retirement Residences is seeking a Health Navigator in Ontario to coordinate resident assessments, care plans, and wellness services within a welcoming retirement community. You will work closely with the Health & Wellness Manager to ensure personalized, high-quality care for each resident, collaborating with residents, families, physicians, and the care team.

The role emphasizes nursing care, medication administration, accurate documentation, and participation in infection prevention

Qualifications

• Must hold an Ontario nursing license (RPN/LPN or RN).
• Strong assessment and clinical skills.
• Experience in retirement living or long-term care preferred.

Responsibilities

• Complete resident assessments and develop care/service plans.
• Coordinate admissions, transfers, discharges, and care conferences.
• Provide nursing care and medication administration as required.
• Collaborate with physicians, families, and care team to address changing resident needs.
• Maintain accurate clinical documentation and monitor wellness outcomes.
• Support infection prevention, quality improvement and regulatory initiatives.

As a Health Navigator at Chartwell, you'll work closely with the Health & Wellness Manager to coordinate resident assessments, care plans, and wellness services. You'll collaborate with residents, families, physicians, and the care team to ensure every resident receives personalized, high-quality care in a welcoming retirement community.

What You'll Do

• - Complete resident assessments and develop individualized care and service plans
• - Coordinate resident admissions, care conferences, transfers, and discharges
• - Provide nursing care, medication administration, and resident education as required
• - Partner with physicians, families, and the care team to support changing resident needs
• - Maintain accurate clinical documentation and monitor resident wellness outcomes
• - Support infection prevention, quality improvement, and regulatory compliance initiatives

What You'll Bring

• - Registered Practical Nurse (RPN/LPN) or Registered Nurse (RN) in good standing with the provincial College of Nurses
• - Current CPR/First Aid certification and medication administration knowledge
• - Experience in retirement living, long-term care, assisted living, or a related healthcare setting
• - Strong assessment, clinical, organizational, and communication skills
