Tuba City Regional Health Care Corporation is seeking a health coach/program coordinator to deliver evidence-based health education for the Take C.H.A.R.G.E. and Lifestyle Balance programs.

Qualifications

  • Bachelor's degree in a health-related field is required.
  • 1 year of health education/coaching experience in preventive health programs.
  • Experience with diabetes prevention programs and behavior change methods preferred.
  • Must have or obtain BLS certification within 6 months of hire.
  • Valid driver's license without restrictions.

Responsibilities

Overview
POSITION SUMMARY

Position is responsible for the delivery of evidence-based curricula to at-risk youth and adults participating in the childhood obesity intervention program, Take C.H.A.R.G.E.; and diabetes prevention program, Lifestyle Balance. Position will provide coaching and support to participants throughout the intervention program utilizing evidence-based strategies. Work duties include the use of health-related assessments and tools to develop health improvement plans for at-risk youth and/or adults. Incumbent will assist the Diabetes Prevention Coordinator in the monitoring and evaluation of health outcomes of participants; and other activities that support the SDPI grant goals and objectives. Understanding and experience with electronic health records. Knowledge of local tribal philosophy and cultural practices is a must. Incumbent must be willing to work evenings and weekends, as needed. This position is grant funded.

Qualifications

NECESSARY QUALIFICATIONS

Education:

Bachelor’s degree in Exercise Science, Physical Education, Kinesiology, Nutrition, or similar health-related field

Experience:

  • One (1) year demonstrated experience in the area of health education or coaching principles, practices, methods, implementation, and evaluation of preventive health service programs.
  • Must have one (1) year experience in diabetes prevention programs for at-risk youth /or adults or related health programs.
  • Demonstrated experience in behavior change, motivational interviewing, basic nutrition and fitness principles

Licensure/Certification

  • Must have and maintain a Basic Life Support Certification (BLS) from the American Heart Association (AHA) or must obtain within 6 months of hire.
  • Training or Certification in Group Fitness or Personal Fitness or Must obtain within 6 months of hire
  • Must maintain and have an unrestricted current valid state driver’s license

Other Skills and Abilities:

A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers. All employment references must address and indicate success in each one of the following areas:

  • Positive working relationships with others
  • Possession of high ethical standards and no history of complaints
  • Reliable and dependable; reports to work as scheduled without excessive absences.
  • Basic knowledge data collection, analysis, practice, and ethical considerations

  • Training in Lifestyle Balance curricula and other evidence-base prevention curriculum for at-risk youth and/or adults.
  • Completion of and, above-satisfactory scores on all job interviews, demonstrating to the satisfaction of the interviewees and TCRHCC that the applicant can perform the essential functions of the job
  • Successful completion of and positive results from all background and reference checks, including positive employment references from authorized representatives of past and current employers demonstrating to the satisfaction of TCRHCC a record of satisfactory performance and that the applicant can perform the essential functions of the job
  • Successful completion of fingerprint clearance requirements, physical examinations, and other screenings indicating that the applicant is qualified to be employed by TCRHCC and demonstrating to the satisfaction of TCRHCC that the applicant can perform the essential functions of the job
  • Submission of all required employment-related documents, applications, resumes, references, and other required information free of false, misleading, or incomplete information, as determined by TCRHCC.
Responsibilities

ESSENTIAL FUNCATIONS

  1. Provide health coaching and support to Take C.H.A.R.G.E. Program and Lifestyle Balance Program participants through the delivery of evidence-based curricula in individual or group settings.
  2. Incorporate and integrate culturally appropriate concepts, modifications and activities when delivering health promotion/diabetes prevention education and services.
  3. Administers initial and follow-up health assessment to include and may not be limited to the following: blood pressure, pulse readings at rest, during and post exercise; FitnessGram administration; aerobic capacity testing, body composition measurements, and tests for muscular strength, endurance and flexibility. Other tests may be conducted to monitor blood glucose levels prior to, during and post exercise sessions.
  4. Assist with referrals and scheduling coaching sessions with all patients/participants of Take C.H.A.R.G.E. and Diabetes Prevention Program clinics and those referred from other clinics/programs of TCHRCC.
  5. Assist in reminder calls and follow-up coaching communication as needed to support participant recruitment, participation, and retention.
  6. Assist participants in setting personal health (health-related fitness, weight loss, etc.) goals and motivate them to adopt healthy behaviors. Utilizes active listening skills to identify issues/needs and provides appropriate solutions or referrals.
  7. Orient and instruct individuals in proper use of exercise equipment and techniques in exercise. Develops and ensures safe environment for participants and community members utilizing fitness center.
  8. Identifies and utilizes appropriate resources for the participants. Works closely with the Diabetes Prevention Coordinator and other health coach/es in the delivery of Take C.H.A.R.G.E. and Lifestyle Balance Program classes meet health outcomes.
  9. Assist with Take C.H.A.R.G.E. Program and Lifestyle Balance Program registry for grant and health outcome reports.
  10. Applies knowledge of behavior change theories and models to meet participant health goals.
  11. Works with care team (medical providers, health coach/es, etc.) on delivery of intervention plan for participants in the Take C.H.A.R.G.E. Program and Lifestyle Balance Program.
  12. Attend meetings as it relates to the grant or secondary prevention programs.
  13. Utilize appropriate media/marketing techniques in delivery of programs to increase program visibility. Assist with media services/social marketing.
  14. Deliver effective communication toward supervisor, program staff, program partnerships, hospital staff, and community members. Communication abilities include written and verbal skills that demonstrate the ability to motivate or encourage others to alter lifestyles and group dynamics.
  15. Foster partnerships within TCRHCC as well as with outside organizations. Provide assistance to other Service Unit HP/DP activities. Works with other departments and/or clinics for collaborative purposes and to meet common goals.
  16. Provides outcome reports and analysis for presentation; includes mandatory reporting.
  17. Understands and adheres to established compliance, safety, and quality assurance standards.
  18. Assists Community Connectors by providing distribution of food, water and other necessary essentials to community members who are in isolation or quarantine.
  19. Assists the Contract Tracing team by performing contact tracing activities for patients testing positive for COVID-19, and conducting negative results calling.
