About the Opportunity

The Health Care Social Worker is a member of the interdisciplinary health care team providing collaborative, holistic care on the primary to quaternary care continuum. Social Workers are primarily concerned with the relationships between people and their environments, which are integral to psychosocial, emotional and spiritual wellbeing. The social determinants of health are embedded in social work theory and practice and are the focus for the dynamic and interactive process of engagement, assessment, planning, intervention and evaluation. Social Workers build upon the strengths of patients/clients by focusing on existing coping strategies and problem-solving skills, past successes, and personal resources.

About You

We would love to hear from you if you have the following:

  • Master of Social Work Degree from an accredited university
    • If there are no candidates who meet the above qualification, those enrolled in a master's program could be considered
  • Registered or eligible for registration with the Nova Scotia College of Social Workers (NSCSW)
  • Minimum of two years clinical experience required
  • Evidence of extensive knowledge of community resources
  • Evidence of ability to work independently with a multidisciplinary team as well as with both hospital and community-based services / agencies
  • Evidence of ability to coordinate / develop / document and implement care plan for hospital discharge and safe transition back to the community or institutional setting
  • Demonstrated use of innovative problem solving and application of evidence-based practice to patient services
  • Knowledge of Joint Discharge Operations Committee and its purpose
  • Competencies in other languages an asset, French preferred

Minimum two (2) years of experience required. If not available, candidates with less than two (2) years of experience may be considered for the selection process. If offered employment, they will be placed on the Health Care Social Worker (Developmental) scale.

  • Permanent Full time /75 Hours Bi-weekly
  • Shifts may include evening ,weekend and holidays
Compensation and Benefits

$40.18- $54.14 Hourly

Developmental ( less than 2 years experience)$35.83-$46.37 hourly

Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
