Job description

Hourly rate: $30-$36 commensurate with experience and based on L’Arche Canada compensation policy

Duration: Permanent, full-time

Position Summary:

We are seeking a full-time Health Care Coordinator. Working within a team context, the Health Care Coordinator ensures the health and wellbeing of members with intellectual disabilities (Core Members) through education, supervision and collaboration around health and medical issues. They also participate in planning for appropriate support and services to meet the ever-changing needs of Core Members and the assistants who support them. The person in this role is accountable to the Community Leader/Executive Director.

Major Responsibilities:

  • Work with home and program teams to ensure appropriate care, planning and consistency around the medical needs of Core Members;
  • Ensure that assistants receive orientation, education and supervision on medication administration and management, medication charts, day to day medical procedures, medical equipment use, controlled acts, matters of good health, and managing medical conditions;
  • Work in collaboration with medical professionals, agency support coordinators, pharmacies, L’Arche Ottawa personnel, Core Members and their families around health and medical concerns;
  • Ensure individual Behavior Support Plans (BSP) are implemented and kept updated including annual signing off. Interface with BSP committee. Records protocols around PRNs within the community and for day programs, when appropriate;
  • Ensure that health care directions and follow up appointments are carried out. Ensures the medical needs of each Core Member are addressed including proper medication storage, dispensing, recording and knowledge of each medication in use. Ensure that specialist appointments are well prepared for, and appropriate accompaniment is provided for these appointments;
  • Document all medical contacts, concerns, appointments, and long-term care applications. Ensure that all health consultation records and binders are accurately maintained and up to date, with appropriate medical information in case of emergencies;
  • Participate to the Health and Safety Committee of the community, staying up to date for all directives and implementation of the compliance rules related to health;
  • Participate to professional networks about intellectual disability;
  • Meet regularly with the Homes Coordinator for updates, sharing information and planning together. Be accountable and keep Leadership informed on a regular basis;
  • Carrying the emergency phone in rotation basis with other coordinators;
  • Any other tasks or activities according to the needs of the organization, and determined by the executive director.

The ideal candidate will possess the following:

  • Registered Nurse or registered practical nursing certification preferred, or significant health care experience and aptitude for medical information;
  • Bilingual (English and French);
  • Demonstrated experience and passion for supporting individuals with intellectual disabilities and individuals with complex behavioural support needs;
  • Demonstrated ability to fully implement and monitor behavioural support plans and supporting documents;
  • Strong interpersonal skills especially communication and advocacy;
  • Ability to be flexible and manage the stress of multi-faceted responsibilities and management;
  • Ability to remain calm, focused and clear in times of crisis;
  • Strong organizational skills and computer capacities. Organized and able to set priorities and follow-through on commitments in timely fashion;
  • Collaborative, excellent communication skills, including the ability to give direction, report information and model effective communication techniques/strategies with the Core Members in the home;
  • Ability to effectively manage multiple tasks, delegate and supervise;
  • Flexible in terms of approach and time;
  • Full G Driver’s License and a minimum of 3 years insured driving experience with insurance records;
  • Maintain automobile insurance with third party liability coverage of a minimum of $2,000,000, and coverage for transportation of Core Members without limitation.

Working Conditions and Physical environment:

  • Will need to work in an office environment with a number of other people, sometimes sharing equipment and space, with possibility to work remotely occasionally.
  • Will need to lead and participate in several meetings, both group and individual.
  • Is expected to participate in meals in the homes/programs and occasionally attend team meetings of each home/program.
  • Some travel may be required.
  • Other specific requirements may be expected.

Salary: Based on L’Arche Canada’s Compensation Policy. Hourly rate starting at $30.93, based on previous experience, skills, and education. Full time employees will be entitled to the Group Insurance benefits including life, dependant life, basic and occupational Accidental Death or Dismemberment (AD&D), Extended Health Coverage (EHC), Long Term Disability (LTD) as well L’Arche’s Employee Assistant Program (EAP) and the group retirement saving plan (RRSP).
