Qualifications

• Graduate of Health Care Aide program (or equivalent with 500 hours theory & practicum).
• Experience working with seniors in community health services.
• Strong written and oral communication skills.

• Sound skills in providing personal care duties.

Responsibilities

• Provide resident-focused care under supervision of a Licensed Practical Nurse.
• Observe residents for changes in status during activities and report to Staff Nurse.
• Contribute to Nursing Care Plan by sharing observations and needs.
• Collaborate with the care team to address resident needs holistically.
• Assist residents with mobility and community activities.
• Report unsafe activities that may cause harm to residents.
