The Geriatric Specialist for the Nursing Led Outreach Team (NLOT) demonstrates a high level of autonomy and expertise in geriatric care to formulate clinical decisions. The Geriatric Specialist on the NLOT appropriately supports geriatric patient care for patients from Long Term Care Homes(LTCHs) within the catchment area with acute/chronic illness and promotes wellness and optimization of their current health condition while utilizing its expert assessment skills to ensure patients receive the best care. The principal responsibilities include utilizing and demonstrating a comprehensive theoretical knowledge base level of clinical competence in caring for elderly patients. The Geriatric Specialist NLOT collaborates and consults with the interprofessional team within the LTCHs and in hospital to ensure comprehensive assessment, analysis, treatment, planning and evaluation of resident’s care and capacity building with staff to improve the quality and scope of care provided to residents in LTCHs.
