Job summary

Extendicare Inc. is seeking a Social Worker to maximize residents' functioning within rehabilitation, maintenance, or palliation and to support problem solving for residents and families.

You will work with a multi-disciplinary team, coordinate admissions, and advocate for resident-centered care. You will also provide education, develop social work programs, and contribute to quality management while maintaining ethical standards and integrity in all interactions.

Qualifications

• Must be registered as a Social Worker under the Health Professions Act, 2000.
• Graduate of a recognized school of social work with a Bachelor of Social Work degree.
• Experience in working with Geriatrics.
• Familiarity with the multi-disciplinary team concept and teamwork.
• Ability to initiate and direct own departmental activity.
• Must provide a current Vulnerable Sector Check or willingness to obtain one.

Responsibilities

• Receive and respond to inquiries related to placement, policies and procedures.
• Conduct home tours with applicants and advocates.
• Coordinate pre-admission procedures including screening and visits.
• Assist with admissions and accompany residents and families through process.
• Promote resident-centered environment and advocate for residents/families.
• Plan and deliver social work programs to meet psychological needs.
• Participate in multi-disciplinary care planning and program development.
• Organize family and resident conferences and care conferences.
• Provide education and consultative services to staff and families.
• Document and report on quality-management data and trends.
