Job summary

Dilico Anishinabek Family Care in Thunder Bay, ON seeks a Certified Diabetes Educator to deliver diabetes public health education to First Nations communities. You will lead monthly education sessions, coordinate quarterly screening clinics, and develop materials to address service gaps.

The role requires a BScN, diabetes educator certification, CPR and standard first aid, and willingness to work flexible hours with on-call rotation. Travel and a valid Ontario license are required.

Qualifications

  • BScN degree is required.
  • Diabetes Educator Certification is required.
  • CPR and Standard First Aid certifications are required.
  • Registration with the College of Nurses of Ontario must be shown.
  • Willingness to work flexible hours and provide on-call service rotationally.
  • Travel regularly with a valid Ontario driver’s license and access to a vehicle.

Responsibilities

  • Deliver diabetes public health education to First Nations communities.
  • Coordinate monthly education sessions and quarterly screening clinics.
  • Research trends and prevalence of diabetes to address service gaps.
  • Develop and/or procure educational materials for community education.
  • Facilitate access to secondary and tertiary care when necessary.

Position Overview

Under the direction of the Community Health Service Manager, the Certified Diabetes Educator will be responsible for the delivery of diabetes public health and education to First Nations communities. This will include the delivery of monthly education sessions, the coordination of quarterly screening clinics, researching current trends and prevalence of diabetes, reviewing, identifying, and addressing service gaps with respect to diabetes intervention, development and/or purchase of educational materials, and facilitating access to secondary and tertiary care where necessary. The Certified Diabetes Educator will positively impact the lives of the community members of the First Nation, provide community members with the education needed to manage their health, and provide current and research-based education to community members and serve the community by promoting positive health care changes.

Qualifications and Requirements
  • A BScN Degree is required
  • Good standings with the College of Nurses of Ontario
  • Diabetes Educator Certification is required
  • Diabetes/Advanced foot care certification is an asset.
  • Three (3) years' experience in community-based service delivery, knowledge and experience in the delivery of public health and education programs, previous experience in the delivery of diabetes education.
  • Must be willing and able to work flexible hours, provide on-call service on a rotational basis
  • Must be certified in CPR and Standard First Aid
  • The regulated health care provider must provide evidence of registration with the appropriate regulatory body
  • Must possess be able to travel regularly, have a valid Ontario Driver’s License and have access to a personal vehicle
