Job summary

The Salvation Army International in Hamilton, Ontario is hiring a Community Support Worker-FFS to provide outcome-based supports to adults with developmental disabilities. You will implement service plans, assist with daily living tasks, and help participants access education, recreation, and employment placements.

This role requires strong interpersonal skills, a valid G license, two years of relevant experience, and current First Aid, CPR with Defibrillation, CPI, and a vulnerable sector

Qualifications

  • Post-secondary program in Developmental Social Work or related field.
  • Minimum two years' experience with adults with developmental disabilities.
  • Current First Aid, CPR with Defibrillation, and CPI certification.
  • Current Criminal Records Check (VSS).
  • Valid Ontario Class G driver's license and access to a vehicle.
  • Ability to lift up to 20 lbs and travel as required.

Responsibilities

  • Provide outcome-based supports per service plans.
  • Assist with health needs (medical, physical, emotional).
  • Assist with personal care and lifting/transfers.
  • Support goal-setting and progress with Service Users.
  • Coordinate with FFS team and Supervisor.
  • Arrange transportation for activities.
  • Promote independence and quality of life.
  • Maintain confidential records using AIMS and complete reports.
  • Stay informed on community programs and services.

Job description

Overview

The Community Support Worker-FFS (Fee-For-Service) provides community-based supports to adults with disabilities. The focus is on life skills, education, recreational activities, and supported volunteer and employment placements. Supports are individualized, outcome based, and designed to achieve goals set by Service Users and their families.

Responsibilities
  • Provide outcome-based support following the plan developed and purchased by the Service User and their family.
    • Ensure all Service User goals, including health (medical, physical, emotional), are met.
  • Provide personal care such as changing incontinence pads and assisting with lifting/transferring a Service User from chair or wheelchair.
  • Assist Service Users with goal‑setting and progress toward meeting their goals.
  • Work cooperatively with the FFS team members and Program Supervisor to provide coordinated and consistent support.
  • Arrange appropriate transportation for each Service User to promote safe travel to activities.
  • Promote greater independence and strive for the highest quality of life possible.
  • Use appropriate behavior modification techniques (e.g., NVCI) as required.
  • Accompany Service Users to groups, workshops, community programs, recreational activities, volunteer placements, and employment placements (in person and virtually).
  • Maintain accurate daily activity records using the agency case management software (AIMS) and complete required reports (shift reports, incident reports).
  • Research locations and services that align with purchased Service User plans.
  • Maintain confidentiality of all Service User information and secure storage of activity records.
  • Stay up‑to‑date on community programs, events, and services that meet Service User needs.
  • Perform additional duties as assigned.
Management and Relationship Responsibilities
  • Reports directly to the Program Supervisor or designate.
  • Provides mentoring and orientation to new staff, volunteers, and Service Users.
Working Conditions
  • Flexible scheduling required; may include evenings, weekends, and holidays.
  • Substantial portion of shifts during weekdays.
  • Ability to lift up to 20 lbs.
  • Travel required (including HSR, DARTS, bicycle, etc.).
Education, Experience, and Qualifications
  • Post‑secondary or college program of at least two academic years in Developmental Social Work, Human Services, or equivalent.
  • Current First Aid, Cardiopulmonary Resuscitation with Defibrillation Training, and Non‑Violent Crisis Intervention (CPI) certification required.
  • Current Certified Criminal Records Check with vulnerable sector screening.
  • Minimum two (2) years of related experience working with adults with developmental disabilities and vulnerable populations.
  • Experience implementing individualized program plans, lifting and transfer training, personal care support, and medication administration training considered assets.
  • Valid Ontario Class “G” Driver’s License, own vehicle/insurance, and recent driver abstract.

Compensation and Benefits
  • Salary range $24.47–$30.59 per hour, with a maximum of $36.71.
