Job summary

St. Joseph's Health Hamilton is seeking a Registered Nurse for the Forensic Outpatient Program at West 5th Campus in Hamilton. You will provide evidence-based nursing care for patients with complex mental illness and participate in a multidisciplinary team.

The role requires extensive mental health and forensic experience, CPR and CPI, with a focus on trauma-informed care and safe, person-centered practice.

Qualifications

• Current registration with the College of Nurses of Ontario.
• CNA certification in Psychiatric/Mental Health Nursing (CPMHN(c)) or commitment to complete within 5 years.
• Current CPR certification is required.
• CPI – Crisis Prevention Training required.
• Minimum of three years of recent mental health nursing experience with serious mental illness.
• Knowledge of Mental Health Act, Consent and Capacity, and related legislation.

Responsibilities

• Provide holistic psychiatric/mental health nursing care.
• Complete assessments, plans and organize treatment/therapeutic interventions.
• Document and evaluate interventions; maintain accurate records.
• Collaborate with an inter-professional team and coordinate care.
• Lead and support within the team; participate in discharge and transition planning.
