Job summary

Manitoba Chiefs seeks Jordan's Principle Advocate/Support Worker to support First Nations children, youth and families in care planning and referrals. Roles collaborate with health and community partners to resolve concerns and ensure confidential handling of client information.

Three full-time term positions based in Winnipeg, with emphasis on cultural awareness and PHIA compliance, serving as a bridge to federal and provincial services.

Qualifications

• Post-Secondary degree in Social Work or equivalent.
• Experience with First Nations communities and complex medical conditions is an asset.
• Knowledge of PHIA structure and adherence to policies.

Responsibilities

• Support clients and families in establishing resolution processes and referrals.
• Explain health care interventions, plans and learning needs to clients.
• Involve clients in developing care and support plans.
• Provide and share referral information with community organizations when needed.
• Collaborate with service providers for issue resolution.
• Maintain confidentiality per PHIA standards.
• Prepare, open and close client case files.
