Job summary

Spring Fertility in Ontario, Canada is seeking a dedicated Patient Navigator to join our care team. You will collaborate with nurses to provide patients with clear, compassionate information as they navigate treatment cycles.

You will generate schedules, track tests, build trust with patients, explain complex medical terms, monitor readiness, and help patients book appointments, ensuring a smooth transition to nursing for cycle management.

Qualifications

• Bachelor’s degree required and passion for reproductive healthcare.
• Exceptional communication skills and patient-centered approach.
• Ability to explain complex medical information in simple terms.

Responsibilities

• Generate journey maps and calendars based on treatment plans.
• Facilitate, track and document completion of tests and diagnostic procedures.
• Develop trust and positive rapport with all new patients.
• Explain complex medical processes in digestible terms.
• Monitor patient compliance and readiness for treatment.
• Schedule appointments.
