Position Details

We are hiring a Family Service Worker for our Gender Affirming Health Clinic. Join us at CMHA-YRSS where innovation meets community mental health care!

Closing Date: April 25, 2025

Salary: $31.01 per hour

Type: Part-time, 17.5 hours/week, evenings and weekends required

Location: The role involves a mix of office work, community visits, and occasional remote work. Travel within York Region & South Simcoe is required, especially to Aurora at 15150 Yonge Street.

Job Profile

The Gender Affirming Health Clinic provides culturally competent, gender-affirming care to trans, non-binary, and gender-diverse individuals. The team collaborates with community partners to ensure comprehensive support, using a holistic and compassionate approach.

The Family Service Worker supports clients' families, focusing on gender identity, transition-related support, and mental health. Responsibilities include assessing needs, developing support plans, psychoeducation, system navigation, and supporting recovery efforts in collaboration with the team.

Key Responsibilities

• Assess family needs with clients and case managers
• Create and implement support plans
• Provide counseling and education to family members
• Facilitate family and client groups
• Maintain a caseload, including travel as needed
• Collaborate with multidisciplinary teams and community resources
• Conduct initial psychosocial assessments and provide ongoing support
• Ensure confidentiality and cultural sensitivity
• Participate in supervision and professional development

Requirements

• Degree or diploma in social work, counseling, psychology, or related field (B.S.W. or M.S.W. preferred)
• Registration with OCSWSSW or relevant regulatory body
• Experience with 2SLGBTQ+ communities
• At least 3 years supporting individuals and groups
• Experience with community health or social services
• Bilingual skills (e.g., Cantonese, Farsi, Mandarin, etc.) an asset
• Experience with families and early intervention is an asset
• Knowledge of mental health issues, systemic barriers, and relevant legislation
• Valid driver’s license and insurance
• Vulnerable Sector Screening

This position reports to the Manager, People and Teams.

Working Conditions

Office, community settings, home visits, and travel required.
