The position requires a CFPC-accredited residency and an Ontario medical licence in good standing, with collaboration across nurses, social workers, and other providers. Immediate start.

Background

Auduzhe Mino Nesewinong (Place of Healthy Breathing) is an Indigenous‑led primary health care initiative that has supported First Nations, Inuit and Métis (FNIM) community members for more than 5 years. Grounded in Indigenous values, self‑determination, and relationship‑based care, Auduzhe was established to address systemic barriers experienced by FNIM peoples. Through trusted community partnerships and outreach‑based service delivery, Auduzhe has grown into an interdisciplinary primary health care model that provides culturally safe, trauma‑informed, and wholistic care across the continuum, including health promotion, disease prevention, mental health and wellness, and primary care services. Embedded within Na‑Me‑Res (Native Men’s Residence), Auduzhe also supports Indigenous men residing at the shelter by delivering culturally safe, trauma‑informed primary health care and mental wellness services in a familiar and trusted setting. Auduzhe’s approach prioritizes community‑identified needs, continuity of care, and coordination with Indigenous and allied service providers, while advancing Indigenous ways of knowing and being within primary health care settings.

Main Function

Auduzhe is seeking a part time Family Physician to join our Indigenous‑led interdisciplinary primary health care team. The successful candidate will have experience working with First Nations, Inuit and Métis (FNIM) communities and a demonstrated commitment to providing culturally safe, trauma‑informed, and wholistic care, relational primary care. This role supports individuals, families, and communities with complex mental health and wellness needs, within a collaborative, community‑based primary care setting.

This role is primarily focused on supporting First Nations, Inuit and Métis (FNIM) community members, including individuals who are vulnerable, structurally marginalized, and/or experiencing homelessness or housing insecurity. Candidates must demonstrate a strong commitment to culturally safe, trauma‑informed, and anti‑oppressive practice, and an understanding of the impacts of colonialism, intergenerational trauma, and social determinants of health.

The ideal candidate will have a minimum of five years lived or work experience in First Nations, Inuit, and/or Metis contexts. They will be an excellent clinician who is confident and competent in providing culturally engaged longitudinal assessment, support, and management of complex social and health conditions such as unmanaged chronic disease, mental health, substance use, sexual health, and the ongoing effects of colonialism. They will be team oriented and will have experience working collaboratively with an interprofessional team to provide outreach and on‑site episodic, transitional, and longitudinal primary care, respecting PHIPA and Indigenous privacy standards. Thecandidate will be expected to perform cross‑functional and/or other duties consistent with the job classification, as assigned or requested. The Physician will provide care to individuals across the life span, promoting a wholistic approach to health, preventing illness and assisting individuals, families and groups to achieve an optimal state of health and wellbeing. The Physician will work in collaboration with the interdisciplinary team including Registered Nurses, Community Health Workers, Social Workers, Physicians, and Nurse Practitioners.

Duties and Responsiblities

The Family Physician will provide assessment and treatment to First Nations, Inuit and Métis (FNIM) community members, while working collaboratively within an interdisciplinary Indigenous‑led primary health care team. Services are delivered in both community‑based and on‑site shelter settings, in close coordination with primary care, mental health, and social service partners.

• Demonstrated commitment to and awareness of Indigenous Traditional Healing practices;
• Excellent communication and interpersonal skills with the ability to work in an interdisciplinary team and foster a positive work environment;
• Strong administrative and organizational skills;
• Proficiency in the use of computers and various software applications;
• Demonstrated ability to work with the Indigenous population and sensitivity to Indigenous issues;
• Demonstrated self‑awareness, in terms of understanding their own culture, identity, biases, prejudices, power, privilege and stereotypes.

Qualifications

Essential Qualifications:

• Successfully completed a two‑year post graduate residency program accredited by the College of Family Physicians of Canada (or the equivalent thereof).
• Licensed by the College of Physicians and Surgeons of Ontario and is in good standing.
