Job summary

Pinecrest-Queensway Community Health Centre is seeking a Family Physician to join our Integrated Healthcare team on a regular part-time basis. You will provide comprehensive primary care in a multidisciplinary setting to residents of PQCHC.

The position requires an MD, CPSO licensure in good standing, CCFP certification or eligibility, and CMPA coverage, with a minimum of two years in family medicine. Bilingual English/French is an asset; flexible hours including some evenings are expected.

Qualifications

• MD degree and current CPSO licensure in good standing
• CCFP certification or eligibility with the College of Family Physicians of Canada
• CMPA coverage required
• Minimum two years of family medicine experience
• Experience with PQCHC priority populations (immigrants, refugees, 2SLGBTQ+ etc.)
• Community-based care experience is an asset
• Ability to work with PS Suite EMR and MS Office
• Flexible hours including some evenings
• Vehicle and G license required

Job Summary

Works with a multi-disciplinary health team to provide primary care services to individuals and families residing in the PQCHC area using a medical care model which emphasizes a holistic approach of prevention, health maintenance, and education.

Job Specific Responsibilities

• Provide comprehensive primary medical care consistent with the standards of practice of the College of Physicians and Surgeons of Ontario (CPSO).


• Diagnose, treat, and manage acute and chronic medical conditions across the lifespan.


• Collaborate with nurse practitioners, registered nurses, allied health professionals, and social services staff to deliver team-based primary care.


• Conduct comprehensive health assessments, develop and review care plans, and provide direct clinical services.


• Order and interpret diagnostic investigations, prescribe medications, and coordinate referrals to specialists and community resources.


• Educate clients on disease prevention, health promotion, and chronic disease management.


• Maintain accurate and timely documentation in the electronic medical record in accordance with professional and organizational standards.


• Participate in team meetings, case conferences, quality improvement, and continuing professional development activities.


• Support clients experiencing health inequities linked to the social determinants of health, such as income, housing, and employment.

Qualifications

• Doctor of Medicine (MD) degree and current licensure in good standing with the College of Physicians and Surgeons of Ontario (CPSO).


• CCFP certification or eligibility with the College of Family Physicians of Canada.


• Current CMPA coverage.


• Minimum of two (2) years of experience practicing family medicine, ideally in a primary care or community health setting.


• Demonstrated experience or strong interest in working with one or more of PQCHC's priority populations: newcomers and refugees, older adults, 2SLGBTQ+ and gender-diverse clients, and people experiencing health inequities linked to the social determinants of health, such as income, housing, and employment.


• Experience in a community setting desirable.


• Demonstrated commitment to and knowledge of community-based health care.


• Technology proficiency – PS Suite EMR, Microsoft Office Suite.


• Flexible hours, including some evenings.


• Vehicle and license requirements – Class G.


• Excellent communication and interpersonal skills.


• Linguistic profile: Bilingual English/French is an asset.
