Job summary

SE Health is a not-for-profit social enterprise delivering compassionate community healthcare across Canada. This On-Call role supports after-hours management, staffing and clinical leadership to ensure high-quality client care and strong team performance.

You will oversee operations, mentor multidisciplinary teams, and act as a clinical resource while aligning with SE Health’s standards and patient outcomes.

Qualifications

• RN license in good standing to practice in Ontario.
• Minimum 1 year leadership/management experience preferred.
• Six months community nursing experience preferred.
• Excellent interpersonal and verbal/written communication skills.

Responsibilities

• Provide after-hours management coverage.
• Respond to staffing, scheduling, client, and operational issues requiring immediate action.
• Coordinate replacement staffing and ensure appropriate skill mix.
• Troubleshoot service interruptions and implement solutions.
• Escalate critical issues to senior leadership and external stakeholders.
• Provide coaching, mentoring, and clinical support to multidisciplinary teams.
• Oversee client care plans and ensure they are assessed, updated, and implemented to support quality outcomes.
• Support complex clinical decision-making and escalated client situations.
• Ensure continuity and consistency of care across assigned regions.
• Serve as a resource on clinical standards, policies, procedures, and evidence-based practice.
• Investigate and respond to urgent client, family, employee, and stakeholder complaints and incidents.
• Manage operational, occupational health and safety, and other risks requiring immediate attention.
• Document incidents, complaints, and risk management activities in organizational systems.
• Participate in root cause analysis and corrective action planning.
• Monitor service delivery for compliance with professional, regulatory, organizational, and health and safety requirements.
• Support quality assurance and client satisfaction initiatives while promoting accountability and continuous improvement
